LIFE PACE Services

Comprehensive Healthcare and Social Support

LIFE PACE offers a variety of services. Based on identified need, the interdisciplinary team works with you and your family to create a plan of care and outlines the services you will receive. 

The plan of care serves as a road map; it is a written plan developed by the participant and the interdisciplinary healthcare team that defines how and when services will be provided. It is formulated by the team after all of the initial assessments are completed. The team then integrates goals that the family and participant would like to achieve. The care plan is reviewed every six months or if a significant change occurs.   

LIFE PACE Services may include the following:



 

Coordinated healthcare is the core of LIFE PACE. Each participant has a team of medical experts dedicated to providing personalized healthcare. When medically necessary, this includes:

  • Multidisciplinary assessment and treatment planning
  • Primary care services 
  • Routine prevention care such as audiology, dentistry, optometry and podiatry
  • Nursing services at home or in the Adult Day Health Center
  • Rehabilitation - physical, occupational, speech and hearing therapies
  • Nutritional counseling and nutritious meals in the Adult Day Health Center
  • Laboratory tests, x-rays and other diagnostic procedures
  • Prosthetics, orthotics, durable medical equipment, eyeglasses and lenses, hearing aids and dentures 
  • 24/7 emergency medical services 


 

LIFE PACE provides medical and personal care in the home to help participants stay independent for as long as possible. In-home care team includes:

  • Aides in the home, if medically necessary, to assist with medication reminders, bathing and dressing, light meal preparation, light housekeeping and other aspects of daily living 
  • Registered nurses and certified nursing assistants, if medically necessary, provide in home medical care
  • Therapeutic home modifications to accommodate disabilities and special needs


 

All prescription drugs and prescribed over-the-counter medications are included in LIFE PACE, at no cost. Other services include:

  • Medication management ensures you're taking the right medications and the right doses, all with the goal of preventing medication errors
  • On-site pharmacy where all prescriptions are filled 
  • Receive medication at the LIFE PACE Center or we'll deliver them to your home


 

 LIFE PACE is both a clinical and social hub for our participants. A team of activity and recreational specialists focus on developing and leading fun and engaging activities, which may include:

  • Arts and crafts
  • Group exercise 
  • Bingo and other games
  • Music and dancing including live entertainment
  • Community outings  


 

LIFE PACE provides transportation to get you from your home to the center and to offsite medical appointments. Our transportation is:

  • Wheel chair accessible
  • Unlimited if approved by your care team
