LIFE PACE provides care to frail elders who want to remain living in the community. Participants are personally cared for by an interdisciplinary team which includes a LIFE PACE physician as well as specialists from a panel of LIFE PACE providers. The interdisciplinary team works closely with participants and families to develop a careplan that represents common goals.
LIFE PACE participants are transported to and from LIFE’s Adult Day Health Center on an as-needed basis to receive healthcare, social services, nutritious meals and to participate in various rehabilitative and recreational activities. Additional services are also available for participants in their homes.

Services must be deemed medically necessary, approved by the team and provided by or authorized by the PACE team (except emergency services). Cost incurred by PACE participants for unauthorized or out of PACE program agreement services may be the responsibility of the participant.
