LIFE PACE Team Partners with Caregivers

A LIFE PACE interdisciplinary team, including a LIFE PACE physician and specialists from a panel of LIFE PACE providers, works closely with family caregivers to develop individual care plans. By coordinating each individual's care needs, the LIFE PACE team provides the helping hands so important to the health and well-being of aging adults. LIFE PACE relies on forming vital partnerships with caregivers, allowing families to be actively engaged in their loves ones care.

Caregiver Support Groups

Support groups provide a safe and confidential space for caregivers to share their personal experiences and feelings, to learn coping strategies, and to troubleshoot difficult issues with others that are in similar situations. LIFE Senior Services caregiver support groups are offered monthly at each of our Adult Day Health locations. These groups are open to the general public and respite is provided at no cost, though registration is required if you plan to utilize ADH respite care.
