Meet the Care Team

At the core of the LIFE PACE program is the Interdisciplinary Team (IDT). With LIFE PACE, you have a team of healthcare professionals working with both you and your family to ensure you get the coordinated care that you need. Your team gets to know you as an individual and helps you with your overall well-being, not just medical care.

When you enroll in LIFE PACE, you will meet with the following professionals:

Primary Care Physician
Center Director
Clinic Nurse
Social Worker
Recreation Therapist
Home Care Nurse
Dietician
Home Health Aide
Occupational Therapist
Physical Therapist
Transportation Coordinator

Specialty Services

If you need to see a specialist, all appointments are handled through your PACE primary care physician and approved by your care team. Our goal is to ensure that you get the best medical treatment and that all necessary parties are involved so that the best-informed decisions can be made.

In addition to your primary care physician, we have many specialists who can provide you with expert medical attention in a variety of fields, including:

LIFE PACE provides emergency support 24/7. If you have a medical emergency after regular business hours, a LIFE PACE nurse is available to assist you.
